Thanks Gena for catching the two "city" fields. I added "or enter 'None'" next to Club Affiliation. And, Linda answered your concern about shirts and dinner fields.

Shirt style (crew or v neck) and size are on the next page, as is the dinner selection, after you sign the form. It shows how many you bought from earlier in the form.
